Advanced Surface Ablation (ASA), also known as Photorefractive Keratectomy (PRK), is a type of refractive laser surgery that is a LASIK alternative. PRK surgery does not create a corneal flap typical of traditional LASIK and can be used to treat nearsightedness, farsightedness, and astigmatism.

ASA is an excellent option for patients with thinner corneas or who don’t like the idea of having a corneal flap.  It can also be used for people who have had previous old-generation LASIK who want to see well again.  ASA provides the same level of vision that LASIK does, but the healing process is a bit longer as there is no flap.

A good candidate for PRK advanced vision correction is typically someone in their 20s or 30s seeking a safe and effective alternative to corrective lenses (glasses or contacts) and would benefit from this LASIK alternative.

How it works

The procedure begins with your ClearSight surgeon using topical anesthetic drops to make sure that you are fully comfortable with the procedure.  The surgeon then gently removes the top layer of the cornea known as the epithelial layer.  Finally the surgeon applies the same laser pattern that LASIK does to your cornea treating your prescription. 

A comfortable no-power contact lens is then placed on your eye to keep you comfortable during the healing process.  Most people can even see an improvement as soon as the procedure is finished!

WHAT ARE THE PROS AND CONS OF PRK LASER EYE SURGERY?

Pros:

  • Patients who are not eligible for LASIK surgery may be a candidate for PRK.
  • There is no corneal flap created during the procedure.
  • PRK is a low-risk procedure with stable outcomes.

Cons:

  • For one to four days after surgery, patients may experience moderate pain or discomfort.
  • PRK recovery can be longer as the procedure takes longer to heal from.
  • Optimal vision takes about one month to achieve.

The vast majority of patients who have the photorefractive keratectomy surgery see a significant improvement in their vision, making PRK a very effective method for correcting eyesight. In fact, if you have 20/20 vision with your glasses or contacts and are a candidate for PRK, you’ll be offered our “20/20 or it’s free” money back commitment on PRK.

PRK  improves vision by permanently changing the shape of  the clear front part of the eye (the cornea). It cannot, however, stop the aging process of the eyes or the potential onset of optical disorders such as cataracts or macular degeneration.

While both procedures are performed with an excimer laser, the top layer of your cornea is removed during PRK surgery, but with LASIK, a flap is produced by cutting through the top layer of your cornea and then replaced once your cornea has been reshaped. This means that most PRK patients experience a longer recovery period than with LASIK or SMILE LASIK. At ClearSight, we usually recommend PRK to those who are not a candidate for LASIK or SMILE LASIK. Many of these PRK candidates may also be candidates for the EVO ICL procedure which offers similar recovery times to LASIK.
On the day of your PRK procedure, our staff will be there for you every step of the way to answer your questions and make sure you’re at ease. We’ll give you eye-numbing drops before the surgery to ensure you’re comfortable. After carefully positioning your eyelashes out of the way, your eye surgeon will prepare your cornea for the procedure. The cornea will next be reshaped, and any flaws that are resulting in your blurry eyesight will be eliminated utilizing the excimer laser. We will put a contact lens on your eye at the conclusion of the surgery to protect it and make you more comfortable as you recuperate. For both eyes, the whole process takes less than 15 minutes.
Most PRK patients report minor stinging and burning in the first few days. Some patients won’t experience any pain. After your PRK procedure, ClearSight will put a bandage contact lens on your eye to help you alleviate discomfort. Additionally, we will provide topical eye drops, often known as comfort drops, which contain a mild anesthetic as well as additional medication to help with any discomfort.
Most patients are instructed to wait one week before wearing eye makeup and two weeks before wearing waterproof mascara. You may feel uneasy at the thought of forgoing makeup, particularly if you often wear it. Applying makeup after a PRK procedure could lead to issues. If foundation powder gets in your eye, it might lead to an infection. However, your doctor will talk to you about this and provide advice based on your particular circumstances.
The occurrence of persistent dry eye following PRK is unusual, although corneal topography of the ocular surface and tear film may aid in making this prediction. There have been numerous studies that show that PRK is not any more or less effective at reducing dry eye syndrome than LASIK.

Most patients have functional vision within a few days but it often takes up to a month to gain optimal clarity after PRK.. Similar to LASIK, most patients see better than 20/20 after PRK; it simply takes a bit longer.

Your recovery from PRK can be shorter and the healing process faster by doing a few things. Following your treatment, it is crucial to take all of the prescribed drops in order to promote healing. Frequently lubricating your eyes with artificial tears might help you feel more comfortable and heal more quickly. We recommend refrigerating your eye drops which will keep them cool and be soothing to your eyes. Rest is also essential for your recovery.After PRK, giving your eyes some rest can accelerate the healing process.
